Many affiliates experience a bottleneck in the referral process because of their dependency on responses of doctors on eligibility and wish authorization. In 2015, a Wish Granting Taskforce, consisting of members of the affiliate community, made suggestions to solve these challenges. One of the suggestions was to change the Medical Eligibility Form, in which the determination of eligibility is still required, while lessening the dependency on doctors.
The Wish Granting Taskforce also discussed the use of the Wish Authorization Form and suggested to only use this form when the wish of the child poses a risk to the child’s health/medical condition, bearing in mind that we already use the Medical Travel Summary Form for travel wishes. Make-A-Wish International subsequently decided to include the requirement for this form for all Phase 1 affiliates, due to their relatively young experience in wish granting.
The changes will be implemented over the course of 2016.
In this document, a summary of the decisions made by the Wish Granting Taskforce is provided, which will help solve the bottleneck in the wish process. This document is based on decisions made after the February 2015 meeting.
The suggested changes by the Wish Granting Taskforce led to some changes in the Medical Eligibility Form where a new definition was given for “Hospital Professional.”
Hospital Professional is defined as a physician, treating hospital nurse, hospital social worker, hospital pedagogical personnel, or hospital psychologist.
