Determining whether a child is eligible for a wish is the start of the Wish Journey. In this section, you will find the International Medical List, which is a compulsory document for all affiliates. You will also find an Interactive Medical Guide that gives descriptions of the most common medical conditions, for internal use. Besides these two documents, we have also added the Eligibility Guidance Sheets that you can amend with your affiliate contact details. These guidance sheets can be helpful for the medical professionals with whom you are working.

We added all the forms we are currently using: the Referral Form, the Medical Eligibility Form, the Wish Authorization Form, and the Parents Form.

When we spoke about the wish granting process with the Wish Granting Taskforce in 2015, we came up with some changes to the current forms and processes. These are also included here. Please note that the draft form and process will be implemented over the course of 2016.
